(4) If something does not work correctly, contact your supervisor or next higher level of maintenance.
(5) Always do your preventive maintenance in the same order.
(6) If anything looks wrong and you can't fix it, write it on your DA Form 2404. If you find something
seriously wrong, report it to organizational maintenance RIGHT NOW.

Explanation of Columns. The
following is an
explanation of
the columns in
table 2-2.
(1)
Item no. This column contains a number for each procedure to be performed. When reporting malfunc-
tions or failures on DA Form 2404, Equipment Inspection and Maintenance Work sheet, place this
number in the TM Item No. column.
Interval. These columns tell you when to do a procedure. Each column that applies will contain a

dot. Some procedures will have dots in more than one column.
Hem to be inspected/procedure. This column contains the name of the item to be inspected and

tells how to perform the required checks and services on it. Carefully follow these instructions and
perform them in the order listed.
(4)
Equipment is not ready/available if:. This column tells you:
Why your equipment cannot be used.
Why there is a problem with any item that was inspected.
What the problem is with the procedure.
